Today, I am going to look at the Code of Ethics in the view of dysphagia assessment and treatment.<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section elementor-element elementor-element-1d7152a elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"1d7152a\" data-element_type=\"section\">\n\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"elementor-column elementor-col-100 elementor-top-column elementor-element elementor-element-911d6d8\" data-id=\"911d6d8\" data-element_type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-4706239 elementor-widget elementor-widget-text-editor\" data-id=\"4706239\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<p><span style=\"font-weight: 400;\">Principle I, Article K of the ASHA Code of Ethics (2016) states that &ldquo;<\/span><span style=\"font-weight: 400;\">Individuals who hold the Certificate of Clinical Competence shall evaluate the effectiveness of services provided, technology employed, and products dispensed, and they shall provide services or dispense products only when benefit can reasonably be expected.&rdquo;<\/span><\/p><p><span style=\"font-weight: 400;\">In her article <\/span><i><span style=\"font-weight: 400;\">&ldquo;Food Culture, Preferences and Ethics in Dysphagia Management &ldquo; (2015),<\/span><\/i><span style=\"font-weight: 400;\"> SLP Belinda Kenny explains an ethical violation with dysphagia: <\/span><b>&ldquo;<\/b><span style=\"font-weight: 400;\">Essentially, two people with the same severity of swallowing impairment may experience very different impacts upon social and cultural participation.&rdquo; (Kenny 2015).<\/span><\/p><p><span style=\"font-weight: 400;\">&nbsp;<\/span><\/p><p>&nbsp;<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section elementor-element elementor-element-1dd0948 elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"1dd0948\" data-element_type=\"section\">\n\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"elementor-column elementor-col-100 elementor-top-column elementor-element elementor-element-1158ab5\" data-id=\"1158ab5\" data-element_type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-33c7ca7 elementor-widget elementor-widget-text-editor\" data-id=\"33c7ca7\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<p>&nbsp;Principle&nbsp;I, Article B in the ASHA Code of Ethics (2016) states that &ldquo;Individuals shall use every resource, including referral and\/or interprofessional collaboration when appropriate, to ensure that quality service is provided.&rdquo; There may be other resources SLPs can, such as communicating with his other health care providers, who might be aware of a patient&rsquo;s cultural preferences and could provide guidance.&nbsp;We also can listen to our patient&rsquo;s information they provide and research dysphagia treatment for culturally diverse populations.<\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section class=\"elementor-section elementor-top-section elementor-element elementor-element-85b3dd4 elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"85b3dd4\" data-element_type=\"section\">\n\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"elementor-column elementor-col-100 elementor-top-column elementor-element elementor-element-60d9a3a\" data-id=\"60d9a3a\" data-element_type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-6984291 elementor-widget elementor-widget-text-editor\" data-id=\"6984291\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t<p>&nbsp;Principle I, Article M that says &ldquo;Individuals who hold the Certificate of Clinical Competence shall use independent and evidence-based clinical judgement, keeping paramount the best interests of those being served. If we are informed of cultural food preferences, and are ignorant of said preferences, whether intentional or not, we are violating the ASHA Code of Ethics Principle I, Article C (2016) which articulates that &ldquo;Individuals shall not discriminate in the delivery of professional services or in the conduct of research and scholarly activities on the basis of race, ethnicity, <span class=\"glossaryLink\"  title=\"Glossary: sex\"  aria-describedby=\"tt\"  data-cmtooltip=\"&lt;div class=glossaryItemTitle&gt;sex&lt;\/div&gt;&lt;div class=glossaryItemBody&gt;Sex refers to the biological differences between individuals that are male-aligned or female-aligned, such as the genitalia and genetic differences. Sex is not the same as gender. I use these terms &amp;lt;strong&amp;gt;&amp;quot;male-aligned&amp;quot;&amp;lt;\/strong&amp;gt; or &amp;lt;strong&amp;gt;&amp;quot;female-aligned&amp;quot;&amp;lt;\/strong&amp;gt; because it is more representative of identities. A reader provided a great example, that not everyone who is female aligned has matching parts. Trans women can be female aligned whether or not they&rsquo;ve had surgery, and folks who have female parts may be male-aligned or non-binary.&lt;\/div&gt;\"  data-gt-translate-attributes='[{\"attribute\":\"data-cmtooltip\", \"format\":\"html\"}]'  tabindex='0' role='link'>sex<\/span>, <span class=\"glossaryLink\"  title=\"Glossary: gender\"  aria-describedby=\"tt\"  data-cmtooltip=\"&lt;div class=glossaryItemTitle&gt;gender&lt;\/div&gt;&lt;div class=glossaryItemBody&gt;The World Health Organization defines this as the result of socially constructed ideas about the behavior, actions, and roles of a particular sex. It is not interchangeable with sex, which is based off genitals. It is decided is based on our internal experience, or &amp;lt;strong&amp;gt;gender identity&amp;lt;\/strong&amp;gt;.A person cannot choose gender identity, but they can choose words to define it.&nbsp; People can identify as male, female, transgender, non-binary. &amp;lt;strong&amp;gt;Genderqueer&nbsp;&amp;lt;\/strong&amp;gt;is term used for those who don&amp;#039;t fit into the identity of masculine or feminine. &amp;lt;strong&amp;gt;Genderfluid &amp;lt;\/strong&amp;gt;represents those who may feel a woman for some period of time, or a man for another period of time. There are many identities that fit into this category such as bisexual, pansexual or polysexual.&nbsp; Sexuality, like gender, is a spectrum and can fluctuate throughout a person&amp;#039;s life. One common measurement of sexual orientation is the Kinsey Scale. There is not an official test, but many exist on the internet. The same opportunity is necessary for patients with dysphagia of all cultural and spiritual backgrounds to experience autonomy and as much pleasure and comfort as possible with eating and drinking. As speech-language pathologists, we need to work with patients and their caregivers to make the best decision regarding diet, taking into account both nutrition and cultural preferences. We have our own perceptions of health with feeding, and must be aware of the potential influence of those beliefs when counseling the patient. Kenny outlines four health professional skills for shared decision making with dysphagia treatment: &ldquo;communication, imagination, courage and reflection&rdquo; (Kenny, 2015). We need to participate in &ldquo;open communication, creative problem solving, leadership, and reflection during patient\/health professional interactions.&rdquo; (Kenny 2015). Beyond what The best practice for a situation like this is to incorporate shared decision making.
